ASSOCIATION HANDBOOK
The goal of the handbook is to provide a framework for the Association as well as address many of the common questions.
PRE-SEASON FORMS
The forms to the right are digitally signed during registration every year. In addition, they will be reviewed in the early-season Parent's Meeting. Please use the documents here as reference.
Additional Forms
Birth Facts forms are required for a first year skater to prove age.
Please use the reimbursement form when requesting reimbursement for pre-approved expenses.
The Financial Aid Request form is used when requesting financial assistance with a skater's registration fees.

CONCUSSION EDUCATION
WAHA and OAYHA are committed to communicating the importance of concussion awareness in the sport of hockey.
Baseline concussion testing is available throughout the year through local clinics. OAYHA will work to set up opportunities for baseline testing prior to the start of the season.
